Registration Instructions from the New York State Department of Health:

Physicians, Nurse Practitioners and Physician Assistants seeking to register with the New York State Department of Health (NYSDOH) to certify patients for medical marijuana must complete a NYSDOH approved Medical Use of Marijuana course. Pharmacists working in dispensing facilities must successfully complete a NYSDOH approved Medical Use of Marijuana course before they may counsel, or supervise another employee who may counsel certified patients and designated caregivers on the use, administration, and risks associated with approved medical marijuana products.

Instructions for practitioners looking to register with the NYSDOH:

Following successful completion of the New York State Department of Health (NYSDOH)-approved course, and upon full compliance with the other requirements set forth in 10 NYCRR §1004.1(a), practitioners who wish to register with NYSDOH to certify patients for medical marijuana must follow the below steps:

Log into the Health Commerce System, Select “All Applications” from the My Content dropdown menu, Click the “M” tab, find the Medical Marijuana Data Management System (MMDMS) application in the list, and click the green and white “+” icon in the “Add/Remove” column to add the MMDMS application. After selecting the MMDMS application, practitioners will be prompted to enter information required to complete the registration process. Please retain a copy of your course completion certificate from the approved course provider for your records.

Arnold Leff, M.D.: Dr. Leff has many years of experience with cannabis medicinal use as well as treatment and policy experience in the area of substance abuse. He was Deputy Associate Director of the White House Drug Abuse office and has served as a drug abuse consultant to the Deputy Secretary of Defense in the past.

Deborah Malka, MD, PhD: Dr. Malka is Board certified in Integrative Holistic Medicine, and has been supportive of patient empowerment and alternative medicine throughout her career.

Dustin Sulak, DO: Dr. Dustin Sulak is an integrative medicine pioneer with a focus in osteopathy, mind-body medicine, and medical cannabis.

Stacey Marie Kerr, MD: Dr. Stacey Marie Kerr earned her M.D. from the University of California, Davis. She completed a Family Medicine residency in Santa Rosa, California and has been board certified by the American Academy of Family Physicians.

John McPartland, DO, MS: Dr. McPartland has earned degrees, residencies, and fellowships at Penn State, University of Illinois, Chicago College of Osteopathy, University of Pittsburgh, and Michigan State University, and has published research on Cannabis, Phyllanthus amarus, Silybum marianum, Serenoa repens, Panax species, and psychotropic mushrooms.

Donald I. Abrams, M.D.: Dr. Donald I. Abrams is Chief of Hematology-Oncology at San Francisco General Hospital and Professor of Clinical Medicine at the University of California San Francisco.
